DIRECT CARE/PARAPROFESSIONAL                      HOURS/PAY VARY

Within the MH/DD/SAS system of care, the paraprofessional is an individual who with the exception of staff providing respite services or personal care services, has a GED or High School Diploma; or no GED or High School Diploma, employed prior to November 1, 2001 to provide MH/DD /SAS services. Upon hiring, an individualized supervision plan is developed and supervision is provided by a Qualified Professional. Starting pay for this position is determined by the President/Director.

Is able to deliver service coordination activities within the established Person Centered Plan, referral linkage, skill building, supportive counseling, and input into the Person Centered Plan modifications. Other requirements include various skill building activities including daily and community interest/activities, symptom management skills, wellness education, educations substance abuse, and work readiness. Transportation to and from service delivery locations is also provided. Documentation in recipients' record is required. Is able to attend training as scheduled. Is able to provide services in a variety of locations to include recipients home, schools, homeless shelters, libraries, etc…

Has and maintains a valid North Carolina Drivers License. Has, maintains and furnishes proof of insurance for the vehicle used to provide transportation for the individual being served. Is in good physical health, lift at least 50 lbs, and is able to obtain a physical determining so prior to beginning employment. Provides a criminal record check and posses no felonies. Hours for this positions are on a strict “as needed” basis, and vary according to the amount of hours allowed to the agency for the individual being served. If the client’s service hours are decreased, the hours are also decreased for the Paraprofessional.
